This article presents the findings of an independent investigation panel, established by the Rebuild Japan Initiative Foundation, to review how the government, the Tokyo Electric Power Company, and other relevant actors responded to the earthquake-tsunami-nuclear disaster that unfolded one year ago. It describes how the players were thoroughly unprepared on almost every level for the cascading nuclear disaster.
